Transcript Requests

Centerville High School maintains copies of grades, attendance, and test results for any student who attended Centerville High School during grades 9-12. We will be happy to mail or FAX a copy of your transcript to any college, place of business or your home address at no charge.

For a copy of your transcript please submit a written request that includes:

• • Full name and/or name used in high school
  • Date of birth
  • Year of graduation or years of attendance
  • Student signature
  • Address of where you would like the transcript to be mailed.
